COMPLEX TRANSITIONS

Specialist clinical and disability support transition team

Maxlife Care has a team dedicated to planning and coordinating a safe and smooth transition from hospital and other residential care environments to your own home.

Our transition teams are led by a Maxlife Care doctor, along with experienced disability support professionals and registered nurses – all experienced in complex disability and medical support.

Our team works with you, your family, the hospital or your existing residential care provider and your multidisciplinary team to:

  • Understand your needs
  • Develop a detailed care and transition plan
  • Find a home that is right for you
  • Progressively transition your care and support until you are safely in your new home

Team based

Enabling you to live your goals and the way you want to live takes a team with diverse and complementary skills and perspectives. We work with your multidisciplinary team to get your support and care plan right and ensure that your support team are trained to support you in the way that works for you. Multidisciplinary team meetings are an integral part of our transition and client management process.

Transition process that pays attention to the details that matter

Our process captures all the important steps that matter for a successful transition. From giving you and the support team the chance to get to know each other as early as possible, through to sourcing, setting up, and training the team and your loved ones on the specific equipment you need to stay well at home. The process is staged so that we make sure that you, your team, and your family are ready when you move into your new home.

Changing Lives

Hear from our wonderful clients

Maxlife Care without a doubt will set a benchmark for care and development of people – they have opened the doors for my family members to train, develop and live a wonderful life.

- Sied

Since Maxlife began helping me with my needs by maximising outcomes from NDIS funding. The staff and wise allocation of resources has eased my life considerably – assistance to remove rubbish and unwanted furniture and the cleaning of my unit. The people at Maxlife are very accommodating and excellent at what they do and I’d gladly endorse them as excellent providers.

- Craig

No company does anything like you do! Stand alone. The barometer by which other companies can aspire to become. Doctors & qualified trained health professionals; they make all the difference when it really matters. What you’ve done for my girls is priceless!

- Jan

The nurse assessing Vicki was so impressed by the care that Vicki has been given. She also mentioned how lucky Vicki was. The warmth and care from your team was and is fantastic. All questions that the nurse asked, were backed up by either forms or lots of verbal answers straight away.

Maxlife should be a role model for everyone to learn from.

- Jodie

They help me step away so I can have a life of my own. Maxlife Care helps me fill that void in my life – giving a bigger purpose to my son. The people that work there are amazing and are like family!
