Maxlife Care has a team dedicated to planning and coordinating a safe and smooth transition from hospital and other residential care environments to your own home.
Our transition teams are led by a Maxlife Care doctor, along with experienced disability support professionals and registered nurses – all experienced in complex disability and medical support.
Our team works with you, your family, the hospital or your existing residential care provider and your multidisciplinary team to:
- Understand your needs
- Develop a detailed care and transition plan
- Find a home that is right for you
- Progressively transition your care and support until you are safely in your new home

We work with your multidisciplinary team to get your support and care plan right and ensure that your support team are trained to support you in the way that works for you. Multidisciplinary team meetings are an integral part of our transition and client management process.

Our process captures all the important steps that matter for a successful transition. From giving you and the support team the chance to get to know each other as early as possible, through to sourcing, setting up, and training the team and your loved ones on the specific equipment you need to stay well at home. The process is staged so that we make sure that you, your team, and your family are ready when you move into your new home.

- Relay Officers are highly trained NRS staff who will assist you to communicate with a hearing person over the phone. They will only translate what you are saying and won’t get in the way of what is being said.
- Depending on the nature of the call, Relay Officers can switch between using text and voice and English and AUSLAN.
- NRS is available to assist you 24/7, except for calls that are made through Video Relay.
- There are different types of Relay services available for you to choose from. This depends on your hearing and speech abilities and the support equipment you may have.
- Please note that in order to use the service, you must register with the NRS.
