Maxlife Care has a team dedicated to planning and coordinating a safe and smooth transition from hospital and other residential care environments to your own home. Our experienced clinical and disability support team will work with you, your family, your support coordinator, the hospital and the multidisciplinary team to understand your needs, find a home that’s right for you, and progressively transition your care and support until you are safely in your new home.

We care for people with a wide range of complex needs, including spinal cord injuries, developmental disorders, intellectual disability, schizophrenia, early-onset dementia, aggression, eating disorders, neurogenerative and neurogenetic diseases and other complex behaviours of concern. 

Along with helping you to build your daily living skills, at home and out-and-about in the community, our support team are trained to provide high-intensity supports including complex bowel care, enteral feeding and management, tracheostomy management, urinary catheter management, subcutaneous injections, and complex wound management.
